I offer a free 15 minute phone consultation (602-625-1414) to discuss your concerns and determine if I might be of help to you. I do not accept insurance, other than for Mayo Clinic employees and their families. Many insurance companies do reimburse for “out of network” providers, and I will provide you with the appropriate billing information for reimbursement purposes. I encourage you to check with your insurance provider before scheduling to find out whether they will reimburse for my services as well as the percentage paid and limits of reimbursement.

If you decide to schedule:

• Therapy sessions - 50 minutes - $140
• Payment can be made by credit/debit card or cash on the day of our appointment
• Please print and complete the following Intake Documents, and bring them to our initial session:
